Telehealth Physicals 

Physical exams provided via telemedicine.

Most appropriate for healthcare workers, school employees or those entering a school or college health program. We do not perform transportation or sports physicals.

If your employer requires vaccinations or PPD please ensure you have these completed prior to scheduling your appointment as you will need to provide proof prior to the visit.

If your physical form requires vital signs ( blood pressure, heart rate, etc) you will need to obtain these prior to your visit as they will be considered self-reported values.

Ages 15+

Payment is due immediately following the visit. 

Completed physical forms will be returned to you within 24 hours from receipt of payment.

It is at the discretion of the medical provider if your physical can be completed via telehealth.  You will not be charged if it is determined the physical cannot be completed.
